Hypertension is a major, rising cardiovascular and stroke risk factor in Nigeria, where limited screening access and healthcare barriers contribute to underdiagnosis. This paper describes the protocol for an effectiveness-implementation hybrid type III cluster-randomized trial testing community-based blood pressure screening at established community gathering places. Two arms are compared: a core intervention combining screening, mobile health support, and facility referral for elevated readings, and a core-plus arm adding community health advisor support for healthcare linkage, with medication vouchers offered to those diagnosed and starting treatment. Outcomes include elevated blood pressure detection, linkage-to-care rates, and implementation feasibility, informing scalable approaches to reducing hypertension-related harm in resource-limited settings.
